Postprandial increases in triglyceride-rich lipoproteins were compared in 10 patients with NIDDM, and 10 nondiabetic controls matched for age, sex, body mass index and plasma triglycerides. Postprandial retinyl palmitate concentrations were higher in patients with NIDDM and correlated inversely with HDL.

An increase in the plasma glucagon level comparable with that seen in patients with NIDDM was associated with lower, not higher, plasma free fatty acid and glycerol concentrations in patients with NIDDM, thus eliminating the possibility that elevations in plasma glucagon are responsible for the higher plasma free fatty acid seen in NIDDM patients.

This article presents lipoprotein data on a population-based sample of Blacks and Whites from the National Health and Nutrition Survey II. In both races, diabetes was associated with higher triglycerides and lower HDL. LDL concentrations compared with nondiabetics were higher in Whites with NIDDM. but lower in Blacks with NIDDM, suggesting that there may be racial differences in the effect of diabetes on plasma lipoproteins.

LDL size and subclass distribution were evaluated in Mexican Americans and non-Hispanic Whites in the San Antonio Heart Study. In both ethnic groups, diabetes was associated with lower mean LDL size and a greater proportion of LDL subclass B. The differences between diabetics and nondiabetics were greater in women.

The rate of the transfer of esterified cholesterol from HDL to VLDL and LDL was evaluated in an in-vitro assay designed to measure the unidirectional transfer from HDL. HDL from NIDDM subjects had a decreased ability to act as a donor of esterified cholesterol, which was related to their higher triglyceride content and lower ratio of free cholesterol to phospholipids.

Lp(a) concentrations were compared in 577 NIDDM patients and 260 normoglycemic relatives. There was no difference in Lp(a) concentrations between the diabetic and the nondiabetic groups. However, diabetic patients with previous myocardial infarction had significantly higher levels of Lp(a) than the patients without a history of coronary heart disease.

Platelet aggregation was reduced, triglycerides tended to be reduced, and glycemic control was not influenced. LDL cholesterol, however, was elevated after EPA administration.

Diabetes Care 1993, 16:584-592. Gemfibrozil was compared with placebo in 60 NIDDM patients in a double-blind study. Mean LDL density decreased and the mean particle diameter increased during gemfibrozil therapy. Changes in LDL composition correlated with decreases in plasma triglyceride.
