HIGM is characterized by elevated levels of serum Igm and a virtual absence of other Ig isotypes. Activated patient T cells either fail to express CD40L or express it in a non-functional form. B cells from HIGM patients show a normal response to recombinant ‘wild-type’ CD40L and secrete IgG, IgA and IgE when cultured in vitro with the appropriate cytokines.
