    • Deletions in the Ligand for CD40 in X-Linked Immunoglobulin Deficiency with Normal or Elevated IgM (HIGMX-1)
    • of outstanding interest, Describes deletions in the gene encoding CD40L in two unrelated patients with HIGMX-1: one has a 59 base pair deletion and the other has an unusual mutation characterized by a C→A transversion at base pair 590 with the deletion of an adjacent C nucleotide. This paper is the first to demonstrate that in some patients (two out of five studied), a CD40L protein, which does not bind CD40, can be detected on the surface of T cells after activation by a mAb against CD40L. This indicates that the loss of CD40L function, i.e. binding to CD40, is the underlying cause of the isotype-switch defect in these patients.

    • The CD40 Ligand, gp39, is Defective in Activated T Cells from Patients with X-Linked Hyper-IgM Syndrome
    • of outstanding interest, This paper describes the investigation of CD40-CD40L interaction in patients with antibody-deficiency diseases. HIGMX-1 patients were found to lack surface expression of CD40L in contrast with common variable immunodeficiency or X-linked agammaglobulinemia patients. Two HIGMX-1 patients were found to have point mutations in the CD40L coding sequence. Soluble recombinant forms of mutated CD40L failed to bind CD40 or to induce B-cell proliferation, whereas B cells from HIGMX-1 patients synthesized IgE in response to CD40 mAb+IL-4. In addition, the CD40L gene was mapped to Xq26.

    • of outstanding interest, This paper describes defects in the CD40L coding sequence in HIGMX-1 patients. Single amino acid substitutions were found in three out of four of the patients studied. The fourth patient had normal coding sequence suggesting that the 5′ 3′coding sequences were responsible for the lack of CD40L expression on the surface. Patient B cells responded to wild-type CD40L or to CD40 mAb.
