In the absence of capped midwifery and physician budgets with fluid boundaries between them, the Ministry of Health would need to assess the degree of functional substitution between these providers, price out the expected net savings, and administratively subtract the savings from the global budgets for hospitals and physician services. A sophisticated analysis would also forecast expected savings from preventive health benefits and prospectively adjust all health program budgets accordingly.
