The percentage of normal children who did not attain a growth hormone value greater than 7 μg/L declined from 61% at pubertal stage 1 to 0% at pubertal stages 4 and 5. Estrogen administration to prepubertal children significantly raised the peak growth hormone response. The false-positive tests for growth hormone deficiency in prepubertal subjects may reflect a prepubertal deficiency of sex steroids, rather than a disorder of growth hormone secretion.

Insulin-like growth factor-I and IGFBP-3 are shown to be useful for screening for complete growth hormone deficiency, but less so for partial growth hormone deficiency and normal short children. A pretreatment IGFBP-3 SD score of less than -1 suggested a favorable short-term response to growth hormone therapy.

N Engl J Med 1995, 333:1093-1098. Mutations in the region of the growth hormone receptor gene that codes for the extracellular domain of the receptor were found in four of 14 patients with idiopathic short stature. These children had normal growth hormone secretion, which suggests partial growth hormone insensitivity due to mutations in the growth hormone-receptor gene.
