    • Physiologic effects of positive end-expiratory pressure and mask pressure support during exacerbations of chronic obstructive pulmonary disease
    • 2O. Application of mask CPAP at a level slightly lower (80% to 90%) than PEEPi significantly reduces inspiratory work load (by offsetting PEEPi) without worsening pulmonary hyperinflation. Application of mask IPPV alone did not offset PEEPi. Tidal volume and respiratory frequency were not affected by CPAP, whereas IPPV caused a significant increase in minute ventilation and improved gas exchange. CPAP and IPPV caused a similar reduction in transdiaphragmatic pressure. The combination of CPAP and IPPV was superior to either CPAP or IPPV alone in reducing transdiaphragmatic pressure and tidal excursion of the transdiaphragmatic pressure.

    • Efficacy of noninvasive ventilation for treatment of acute exacerbation of chronic obstructive pulmonary disease
    • 2O, and expiratory pressure kept at atmospheric level. Improvement in gas exchange was observed within 1 hour in the NPPV group only. Patients randomly assigned to NPPV had a significantly lower intubation rate (26% vs 74%; P < 0.001). Most intubations (74% and 82% in each group) occurred in the first 12 hours. The mean duration of successful NPPV was 4 ± 4 days. Complication rate (14% vs 45%; P < 0.001), mortality rate (9% vs 29%; P = 0.02), and length of hospitalization (23 ± 17 days vs 35 ± 33 days; P = 0.02) were significantly lower in the NPPV-trealed group. Eight-one percent of deaths occurred during MV.
