Surfactant therapy in the adult patient

Gommers D, Lachmann B: Surfactant therapy in the adult patient. Curr Opin Crit Care 1995, 1:57-61. This article describes the rationale for treatment of patients with ARDS undergoing exogenous surfactant therapy. Case reports of surfactant replacement therapy in adult patients are presented. Possible reasons for differences in outcome of surfactant therapy are described, eg, surfactant dose, application technique, and compositional differences in surfactant preparations.

Liquid ventilation improves pulmonary function, gas exchange and lung injury in a model of respiratory failure
-
2 in the 50 to 80 mm Hg range was significantly lower in the TLV animals as compared with the gas-ventilated animals. After discontinuation of ECLS, all gas-ventilated animals died within 1 hour, whereas all TLV animals demonstrated effective gas exchange for 1 hour.

Surgery 1994, 116:159-68. By means of computed tomographic imaging it is shown that in oleic acid lung injury, during gas ventilation, the dependent lung areas are minimally ventilated whereas the nondependent areas remain well aerated. During TLV, there is a homogeneous distribution of perfluorocarbon in the lungs.

This group described the adaption of an ECLS circuit as a time-cycled volume-limited liquid ventilator. They applied TLV with this system to eight healthy cats and four oleic acid lung-injured young sheep on ECLS. They conclude that with this simple adaptation of an existing ECLS circuit TLV can be performed successfully.

Crit Care Med 1993, 21:962-969. First experiment with PLV in animals with induced acute respiratory failure. This study showed a dose-related improvement in gas exchange with increasing doses of perfluorocarbon, whereas lung mechanics already improved with low-dose perfluorocarbon.
