Chest 1995, 107:1083-1088. A survey in 1426 patients with acute respiratory failure treated in 25 clinical centers, 11 in the United States and 14 in Europe. Hospital survival rates for patients in severe acute respiratory failure appeared to be essentially the same as for similar patients cared for 10 to 20 years ago.

A report of a highly standardized, randomized clinical trial of pressure-controlled inverse ratio ventilation and extracorporeal carbon dioxide removal in 40 patients with ARDS. The main outcome measure was survival at 30 days after random assignment. This study failed to prove any survival advantage of the extracorporeal technique.

A prospective study in 27 patients with ARDS to evaluate the behavior of inflammatory cytokines during progression of ARDS and the relationship of plasma inflammatory cytokines with clinical variables and outcome. Plasma interleukin-1 and interleukin-6 levels were consistent and efficient predictors of outcome over time.

A cohort of 918 adult patients admitted in an ICU during a 10-year period (from 1983 to 1993) met ARDS criteria. The analysis of data showed a significant decrease in fatality rates of ARDS since 1987.

The purpose of this study was to evaluate ARDS outcome using this new ventilatory technique, based on retrospective studies by the same investigators. Although this study is a nonrandomized trial, lend further support to the hypothesis that limitation of peak inspiratory pressure and reduction of lung overdistention may reduce ventilator-induced lung injury and improve outcome in ARDS.
