A clinical trial on the prevention of catheter-related sepsis using a new hub model. Ann Surg 1996, 223:363-369. This report showed that in catheters with a mean duration of placement of 2 weeks, the hub is a common portal of entry for bacteria causing subclavian catheter-related sepsis. The use of a new anti-infective hub was shown to be useful in preventing endoluminal bacterial colonization of subclavian catheters and catheter-related sepsis. Although most (93%) cases of catheter-related septicemia were caused by S. epidermidis, molecular typing of strains isolated from blood and catheter were not compared to ascertain the clonal relationship.

Crit Care Med 1996, 24(suppl):A121. In this prospective, randomized, evaluator-blinded, multicenter study, CVCs coated minocycline and rifampin prevented the occurrence of catheter-related septicemia and reduced the rate of catheter colonization more than threefold compared with uncoated catheters. The use of molecular techniques to ascertain the clonal relationship between blood and catheter organisms, the use of scanning electron microscopy to confirm inhibition of ultrastructural colonization of coated catheters, and the techniques used to predict duration of antimicrobial protection provided by coated catheters are major advantages of this study.
